IMSI (Intracytoplasmic morphologically selected sperm injection) for the precise selection of sperm for ICSI:
In comparison with the conventional enlargement under a microscope in ICSI (approx. 300-fold), special microscopes (e.g. the Leica AH 6000) provide a 6000 to 8000-fold magnification for the selection of sperm for intracytoplasmic sperm injection (ICSI).
With the enlargement of the sperm under the IMSI microscope, precise diagnoses can be made about morphological changes in the individual sperm. This is not possible with the normal ICSI microscope in this shape.
Defects in the sperm head area (for example vacuoles) are potentially an expression of a DNA fragmentation.
The fertilisation of egg cells with these sperm may lead to the impairment of the vitality of the created embryos.
When it comes to the decision regarding whether IMSI could be an additional therapy option, a diagnostic spermiogram under IMSI criteria is helpful.
The sperm are classified according to the Cassuto score used by us:
Class 1 = completely unremarkable,
Class 2 = moderately remarkable,
Class 3 = all sperm is very remarkable
If the proportion of sperm in Class 1 is less than 5 %, IMSI is useful for the ICSI technique to increase the possibility of injecting morphologically unremarkable sperm.
